Health Office Requirements

Welcome to South Shore Vocational Technical High School! I wanted to give you some information required by the Health Office that will help make your child’s transition here a smooth one.

All students must submit the following to the nurse for admission to SSVT:

  • Physical Examination (dated within the past year)
  • Immunization History
  • Student Health Emergency Information Card (yellow)

This paperwork can be faxed to 781-499-7495, emailed – cworrall@ssvotech.or or mailed directly to the school. No student may start school without an updated physical or immunization history.

Should your child require daily medication be given during school hours, both parental and physician authorization forms must be filled out. These can be found here on the website in the upper right hand corner. They are labeled “Parent/Guardian Medication Consent” and “Physician Medication Order Form”.

Any changes in health status or any medical concern should be identified. Please contact the Nurse’s Office so we may meet the needs of each student. A Student Health Emergency Information Card should be filled out completely, including parental consents located on the back of the card. These include consents for standing order medications (i.e. Tylenol, Ibuprofen).

All student athletes, regardless of grade level, must have a yearly physical examination per MIAA regulations. It is mandatory to have a current physical exam prior to participating any sport – so please send in proper documentation of the physical to the Health Office on a yearly basis.
